How do you line up the cam and distributor gear?

You have to turn your engine by hand till the timing markes line up dead center. Be carefull that # 1 piston is in the firing position ( all the way up ). You could have marks lined and be 180 degrees off. Set the dist cap on the dist. the cap will have a 1 on top indicating #1 plug wire. Using a marker, mark the side of the dist even up with this mark. Point the rotor at this mark and start slipping the dist in . You will need to back the rotor up an inch or so as it will turn as the gears ingage. I never had one of these out but you may have a slot or flat that needs to ingage. Either way or both. this is the plan. Sometimes you need to use a long screwdriver to line the slot up down in the motor once your aware how the dist shaft is going to line up.

I need to put my cam in first. Wont I have to line up the gear that the distributor goes into with the gear on the end of the cam? How will I know the distributor gear is in sync with my cam.

In my 318 there is a gear on the end of the camshaft that meshes with the gear that has a slot in it for the distributor - the end my distributor does not have a gear but it slips into the gear with a slot -
Dont I have to make sure that this is lined up properly?

It'll either be in, or 180 out when it's in the slot. Ideally, you have the cam and distributor gears mesh so that when the cam slides in, the gear will rest with the slot pointing diagonally towards #1 and #8 cylinder. This is only a convenience in referencing #1 cylinder and TDC. If you're having trouble getting the slot in the right place, slip the distributor in, slide the cam back, and use the distributor rotor to spin the gear a bit. There should be no restriction there, so it's not a load for the plastic rotor to bear. Be sure to do this with the cam timing gear on so as to maintain a reference point for the timing gear points. Anyway, spin the distributor rotor a bit, slide the cam in fully, and see if the rotor is pointing towards #1 cylinder (driver side front of the engine). If not repeat til you get it there. It only takes a couple tries to narrow it down. If, by some chance, you get it all lined up, but 180-out, all you have to do is lift the distributor a little, spin the rotor half a turn, and drop it back into the slot.
In fact, the distributor drive gear and slot can be in any position as long as the cap is wired to accomodate this...meaning, find TDC on #1 and see where the dist. rotor is pointing......that's #1. Wire the cap accordingly and you're done. Lining up the slot to point at #1 on TDC is just an extra reference point, but has no effect on engine performance.
