Scott, my first recommendation is to get yourself a Haynes book for the truck. Second, I have to ask how long have you owned the truck and how long has this been going on. Does it only start when it's cold or won't it start at all any more? Code 12 is "Direct battery input to PCM was disconnected within the last 50 key-on cycles" For other codes go to: http://www.geocities.com/Baja/Desert/1727/FaultCodes.html

It will always run crappy for the first 30-60 seconds after resetting the PCM. It has to learn how to idle properly. My 98 5.2L does the same thing. You just have to feather the petal to keep it from stalling, then it should be OK

I'm a little confused, is it starting or not starting? Let's here the exact problem one more time and we'll go from there.
