Check the fuel pressure. Sounds like it may be the fuel pump.

it sounds like a fuel pump, i had the same problem 1 month ago, just check the fuel pressure if it isn't above 45-60 psi you found the problem.

considering this happened all of a sudden, I'd be willing to bet you slipped a tooth on the timing chain....the fast turning, deep sound and full throttle are indications of this.

Do a compression check on one cylinder. If it is below 100psi, I'd start tearing into the timing chain right away ( you'll be tearing the heads off anyway if it's worse than the chain anyway ).

The fast starter is because it's not being slowed down by the compression in all the cylinders, the noise is because the exhaust valves are open sooner or longer during their part of the 4 stroke cycle allowing compressed air to be forced past them.

Sounds like your distributer sensor plate is fried. check your diagnostic codes on off on off on. If it is an 11 then change your distributor sensor inside your distributor cap under your rotor.

My mother in law works for Autozone and hasn't had anything bad to say about their reman's, and with the mileage on the motor, replacing it with a crate motor wouldn't be a bad idea. IF you love your truck enough, I'd say go for it! ( you may want to have the transmission checked while you are at it, as long as you'll have the motor out anyway.
