Every time that I change the oil on my 89, 3.9 with 167K, I get a very loud knock until I add a blue bottle of STP. The knock is easily heard from 50 ft away, especially when the engine is cold. Am I facing immanent destruction?

but you are filling the oil filter with oil, letting it sit a bit then putting it on
or you just putting the filter on empty and dry?

Empty and dry, but the knock will stay this way for weeks, or forever, until the STP is added, and then the knock is totally gone until the next oil change. I fill the filter on my new truck, but this old one is not street legal and just gets driven through the fields and woods.

Pretty much impossible to say exactly what is knocking. You need to isolate the knock using a spefascope or even a small metal rod available at any hardware store. Almost any knock that can be heard at 50 ft. is in a distructive stage but not all. The STP is just making your oil thicker. What weight oil you using in it? Considering what your using it for, why not try a 20-50 oil. This is assuming your just trying to extend the life of the engine as long as possible.

So many problems .... So little time

I strictly use 10W-30 is all my gasoline engines. 10 for cold starts, 30 should be heavy enough for our hottest weather in nePA, and the smaller the span between the numbers the more lubricant and the less additive eg. viscosity improvers. If it were just a matter of oil thickness, I might expect it not to knock when cold or cool without the STP, or start to knock when hot despite the STP.
