You can use anything in between 3.90 and 4.11. Example 3.92 and 4.10 are the gear choices for one manufacturer and 3.91 and 4.11 are the choices for another manufacturer, the exact ratios can vary a little from company to company. I have a 79 Bronco that is getting 4.10s in the front axle but I can only get 4.09s for the rear axle.

Really depends on what quality gear you buy and how many of the bearings you want to replace if you want to go with cheaper gears (Accu gear or Motive gear) with a minimal install kit look to spend $250-300 on parts, and if you do a very high quality gear (richmond or precision gear) and a full master install kit look to spend upwards of $500 on parts. Labor is going to be worse then that probably in the $500-700 range. Best off if you can't do it yourself to try to find a friend that can or at least a friend of a friend that can.
