Jeff you don't know it yet but you've gone and done it now by asking that.

Your best first mod is to slip on a fire retardant suit and get into a bunker.

As for your truck, no. The TB spacer is worthless on a SMPFI engine. I would go with a CAI first. Save the spacer money for a bucket of KFC and a couple of 6 packs. It'll do more for you.

Spacers work with carbs, not SMPI. The difference is how the fuel is delivered. Spacers increase the effective plenum volume an engine can draw from. This is important with atomization of fuel as seen with carbs, and the flow of the mixture through the intake. SMPI units flow air through the TB and squirt gas at the intake port/valve. No plenum volume atomization situation. Thus, they don't work with SMPI. Chicken sounds good. :-)

This mod wont work correctly without first adding Altezza taillights and a 6" muffler tip.

A TB spacer is good in theory. But in practice it doesn't work. If it did, all the makers/sellers of them would be posting dyno sheets. But none of them do.

For a first mod, use a reprogrammer like SuperChips said. I used a Hypertech on my essentially stock 5.9 and it made a huge difference. A cold air induction will add some power, but probably not a huge amount. Maybe not even enough to notice on the 3.9.

Everyone talks about modifying the exhaust. You won't gain any power there, unless you go with headers and high flow cats. Cat back systems only change looks and sound, they generally don't add power.

If you want better off the line acceleration, change the rear gears. Best bang for the buck.

You should notice better acceleration once you replace your stock airbox and air filter on the v6. The programmer will give you some more power, perfect your timing pulses if you run 91 octane and if you have an automatic transmission it will shift quicker and change were your od kicks in at.

Ummm, cat back systems do help. Naturally, it depends on how good the system is you're replacing, but generally you will pick up some (10-20hp) power. It's not all bad. Obviously, headers back is gonna be your best power-up for exhaust work.

you really think it's possible to gain 20 HP on a 3.9 with only a cat back system? do you write the ads for the throttle body spacer manufacturers too?
