 Subject: fan clutch, help

anybody hear of a fan clutch locking up??? I just did my head gaskets and everything is ok except the fan clutch is completly locked, any rpm and the fan whirrs really loud. Any way this could have happened? bolted it wrong? i thought when they went bad they got loose. Also, will a broken temp. sensor cause the check engine light to go off? the gauge reads nothing, and the engine runs a little rich and keeps the rpms up more? is this why? tghanks for any input.

They lock up sometimes. You need a new one.
