Why do I need a new belt every four months? WTF? I've had the belt replaced several times, and it just starts to squeak like crazy a few months later. Last time, I had the alternator and a loose pulley replaced in December. It's April now, and the squeaking came back, despite the fact that I thought I had fixed the problem. WTF. Why is my truck chewing through belts like crazy? Oh, I also had a new belt tensioner installed.

The tensioner should have corrected the problem; they wear at the pivot point and get out of alignment. The only other thing would be pullys out of alignment.
Your serpantine belt should last the life of the vehicle.

check the alignment of all the pulleys. something is out of wack. unless you idler pulley is bad. everyone thinks about the tensioner but forgets about the poor little idler pulley.

You said a loose pully was replaced. Bet someone boo-boo'd there.
Had a tenioner installed?
If someone was paid to do it, you did pay to have it done correctly. If you didn't pay to have someone install parts, you're paying. Have them re do it.
Got H2O? Grab the fan and do the ol push pull. Movement should be barely detectable. Does it wobble, along with the pully?
As daddio says, is the bolt tight? does it wobble?
Best do the above with the motor off. If it hasn't fallen off remove the belt to check.
