Mine (NV3500) went bad after 50K miles and dealer replaced it under extended warranty. I heard what sounded to be a bad input shaft bearing (grinding/rumbling when in neutral with clutch engaged)and saw leaking lube from bell housing (front seal). Dealership claimed 3rd and 5th gears were shot.

Rebuilt transmission they replaced it with makes the same "bad input shaft bearing" noise. Hmmmm.... My friend has a GMC 1500 with the NV3500 in it and complains his transmission is noisey, also.

Yeap. Mine is a 2001 with the 4.7L. NV3500 and they listed at sales time as a "Heavy Duty" transmission. Look at the description in the factory service manual and it is described as a "medium duty" transmission. I suppose the sales person has his description for th epurpose of sales, and the mechanics have their description for the purpose of replacement/repair.

My wife and I are middle-aged and drive the truck real easy, no hot rodding the family car for us. I thought the manual would lastthe lifetime of the vehicle, being simpler design than an automatic, but appears I was mistaken.

My Datsun 280ZX has a manual transmission needing rebuild. But it has 235,000 miles on it, not 50,000.
